Please read these Terms & Conditions before booking an appointment with Mend & Move Injury Clinic. By booking an appointment, you confirm that you have read and agreed to these terms.
Appointments
Appointments are provided at the agreed date, time and location. Please arrive on time for your appointment. If you arrive late, your appointment may still need to finish at the scheduled time.
If you are unsure which appointment length is most suitable for you, please get in touch before booking.
Cancellations & Missed Appointments
Our cancellation policy requires at least 48 hours notice if you need to cancel or rearrange an appointment. Cancellations or missed appointments with less than 48 hours notice may be subject to a cancellation fee.
Treatment & Consent
Treatment and rehabilitation is tailored to each individual following an appropriate assessment. You are responsible for providing accurate and relevant information about your health, medical history and current condition before treatment.
If you have any reports, scan results or other relevant medical notes relating to your injury, please email these to mendandmoveinjuryclinic@gmail.com or bring them with you to your appointment.
You may ask questions about your treatment and withdraw your consent at any time.
If treatment is considered unsuitable or outside the scope of our services, you may be advised to seek further assessment or care from an appropriate healthcare professional.
Advice & Rehabilitation
Any advice, exercises or rehabilitation provided are intended to support your recovery and should be followed as advised. If you have any questions or concerns about the advice, exercises or rehabilitation provided, please contact Mend & Move Injury Clinic so that these can be discussed and clarified.
Mend & Move Injury Clinic cannot be held responsible for outcomes resulting from choosing not to follow the advice or rehabilitation provided.
Payment
Payment can be made by bank transfer or cash following your appointment. After your appointment, you will receive an invoice along with an email summarising your appointment and the treatment provided.
Confidentiality & Privacy
Your personal and treatment information will be handled confidentiality and in accordance with applicable data protection legislation.
Please refer to our Privacy Policy for further information about how your personal information is collected, stored and used.
Insurance
If you intend to claim the cost of treatment through insurance, please check with your insurer before booking to confirm that your treatment is covered and what documentation they require.
